How to Obtain an Updated Certificate of Immunization Status (CIS)
At Cherubs Nursery School, we require an up-to-date Certificate of Immunization Status (CIS) for all enrolled children. Washington State law mandates that all child care centers maintain immunization records for their students.
Ways to Access Your Child’s Immunization Records
1. Use MyIR Mobile (Recommended)
MyIR Mobile is Washington State’s official online immunization record system, where you can view, download, and print your child’s Certificate of Immunization Status (CIS).
Go to wa.myirmobile.com
Sign up or log in to access your child’s immunization records.
Download and print the CIS – MyIR version, which is accepted by all schools and child care facilities.
💡 If you have trouble accessing MyIR, call the Washington State Department of Health at 833-VAX-HELP (833-829-4357) or email waiisrecords@doh.wa.gov for assistance.
2. Request a CIS from Your Healthcare Provider
If you cannot access MyIR Mobile, you can ask your child’s doctor, clinic, or healthcare provider to print a Washington State CIS form. They can:
Provide a pre-filled CIS showing all recorded immunizations.
Help you update missing or new immunization records.
3. Use the Washington State Immunization Information System (WAIIS)
Contact the Washington State Department of Health to request your child’s immunization records.
Email waiisrecords@doh.wa.gov or call 833-VAX-HELP (833-829-4357) to request a copy.
Submitting Your Child’s CIS to Cherubs Nursery School
Once you have an updated Certificate of Immunization Status, submit it to us using one of the following methods:
✅ Email: Send a scanned copy to cherubswa@gmail.com
✅ In-Person: Drop off a printed copy at our front desk
